Application Process & Forms
In order to be considered for evaluation and treatment in clinic, prospective clients or caregivers need to fill out and submit a case history form to give the clinic some background information about the client. Once this case history form is received, the client will be put on the clinic's waiting list until an opening is available. Click one of the below links to download the case history form.
