MASTER OF PHYSICAL THERAPY - DOCTOR OF PHYSICAL THERAPY (MPT-DPT) APPLICATION

FALL 2010 MPT-DPT APPLICATION PERIOD NOW OPEN.

  1. Two (2) Separate Applications are required:

    1. MPT-DPT Application Instructions for Fall 2010
    Application Available July 15, 2009 via PTCAS
    Firm Deadline: December 1, 2009

    NOTE: It is recommended that applicants applying for the
    MPT-DPT progam meet with Admissions Coordinator, Penny Lacy, periodically throughout the application process.

    2. CSU Graduate Application - Fall 2010
    Available through www.csumentor.edu
    Deadline: November 30, 2009

  2. Verification of Observation Hours Form
    It is REQUIRED to submit a PT Observation Hours form verifying your observation hours for every location and number of hours observed. Login to PTCAS and use the online PT Hours signature process or print each customized PT Observation Hours form, and have your hours verified by a licensed physical therapist. If using the paper verification option, mail the signed form directly to PTCAS. Please read all directions set by PTCAS to avoid any delays in the submission of your application.

  3. Recommendation Letters
    There is NO letter of recommendation form. Arrange for three (3) letters of recommendation to be sent to PTCAS on your behalf; one each from a licensed physical therapist, and a non-Physical Therapy professor. Only original letters will be accepted and must be in a sealed envelope. To see complete list of acceptable reference sources, please our Fresno State's informational page on PTCAS. Please read all directions set by PTCAS to avoid any delays in the submission of your application.

Physical Therapy Application Tips

  1. COMPLETE the GRE exam no later than September 1st!
  2. COMPLETE your 100 observation hours with 50/100 hours in an in-patient setting. Get these hours completed early since hospitals and Physical Therapy clinics can only accomodate a limited number of students. Verification of hours need to be completed when you turn in with your application.
  3. PTCAS Application deadline is a FIRM deadline and on December 1, 2009.
    CSU Graduate Application deadline is November 30, 2009.
