Customer Request Form

For emergency or other repairs which pose an immediate health, safety, or security risk, please call the Work Control Center immediately at (559)278-2373, do not submit a form. If you are experiencing an emergency during off hours please call University Police at (559)278- 8400.

Please be aware, non-maintenance requests may result in a chargeback to the department. An authorization from the department or dean is require on all chargeback work.

Status Checks: If you are doing a follow up on a previously submitted service request please email fmcustomerservice@csufresno.edu for a status check. Do not complete a second customer service request.

NOTE: After you submit your form, you will be sent a confirmation email that requires your approval. Please review your customer service request through this email and click the approval link to have the request sent to the Work Control Center. For any questions please email fmcustomerservice@csufresno.edu.
