Family & Medical Leave
Employees may take a leave of absence for medical reasons or to care for a family member. In addition, an employee becomes eligible to be covered under the Family and Medical Leave Act of 1993 after one year of employment. Eligible California State University, Fresno employees are entitled to Family Care and Medical Leave in accordance with state and federal laws effective February 5, 1994, which, among other rights, grants up to 12 weeks of employer paid health, dental and vision benefits during a 12 month period while the employee is on an unpaid family and medical leave of absence. All leaves require various forms and doctor’s verifications. Please see individual leave types for leave requirements. How to Initiate a FMLA Leave
- Contact Human Resources Joyal Adm. Rm 164, 278-2032 and make an appointment to complete the necessary forms and to get further information. If you are unable to come to the office for an appointment, please call 278-2032 and speak to a Human Resources Generalist for more information. The required forms and doctor’s verifications are listed below.
Requirements for Leave
To participate, an employee must complete the following:
- Family and Medical Leave Notice and Request Form (if you are eligible; please see Your Rights Under FMLA for more information).
- Certification of Health Care Provider
- Request for Leave of Absence Form
- Doctor’s Verification obtained from your doctor
- Attendance Summaries are to be turned in as normal to the Payroll Office detailing how your leave credits will be used. (Note: If you are taking time on an intermittent basis a copy of your attendance summaries must be turned in to the Human Resources Office, Joyal 164, Mail Stop JA71.)
