Leave Programs

Generally, full-time or part-time employees may be granted full or partial leaves of absence with and without pay depending on the circumstances and duration of the leave request. Eligibility guidelines are subject to appropriate Collective Bargaining Agreements (CBAs), specific policies, applicable legal agreements, and current state and federal legislation. There are several types of leave of absences that may be granted to employees depending on the circumstances of the leave and the appropriate collective bargaining unit of the employee.

Employees who are in need of an extended leave from the university that cannot be granted under the provisions of a vacation should review some of the types of leaves available below:

  • Family & Medical Leaves
  • Nonindustrial Disability (NDI) Leave
  • Catastrophic Donated Leave
  • Parental Leave - Maternity/Paternity/Adoption Leave
  • Care for Family Member
  • Long Term Disability Leave (LTD)

How to Initiate a Leave - Employee Responsibility

  • Discuss with his/her appropriate supervisor/manager/chair the request for the leave.
  • Contact Human Resources Joyal Adm. Rm 164, 278-2032 and make an appointment to complete the applicable forms and to get further information. If you are unable to come to the office for an appointment, please call 278-2032 and speak to a Human Resources Generalist for more information.
  • If possible, employees are encouraged to download applicable forms and complete prior to making an appointment.

  • Request for Leave of Absence (Staff & MPP)
  • Personal Leave Application (Faculty)
  • FMLA (request for FMLA)
  • Nonindustrial Disability (NDI)
  • Catastrophic Donated Leave

How to Initiate a Leave - Department Responsibility

  • The appropriate supervisor/manager/chair should consult with the Human Resources Generalist in Human Resources immediately upon learning of a leave or potential leave of absence, to determine if the employee is eligible for Family and Medical Leave, and to review the leave conditions.
