Most Common Preparation and Process Errors

  1. Control Sheets - Frequently control sheets are not filled out properly. Examples of inaccuracies include: 
    • Numbers are transposed.
    • Faculty ID’s do not correspond to the course being scanned.
    • Numbers are not bubbled in, beneath where they are written.
    • Numbers are bubbled in incorrectly. They do not match the numbers written in above them.
    • No control sheet at all.
    • The wrong control number bubbled in the special code section of the scantron form. Example 333333 is bubbled in instead of the default 222222 or for a special grouping 222223. Click here to view more detailed information about how to complete the checklist and control sheets.
    • Using pen instead of #2 pencil.
    • Student answers on control sheet.
  2. Scantrons without Envelopes -  Scantrons have been dropped off without envelopes.  We will not accept evaluations without envelopes. All scantrons must be separated by schedule number and in individual envelopes with the Peoplesoft preprinted course labels or department created labels affixed.
    If you choose to create your own they must have the following information:
    • Department name
    • Schedule number
    • Faculty name
    • Last four digits of the faculty SSN
  3. Special Groupings - Scantrons do not indicate a special grouping code; however, the special grouping code is indicated on the checklist. When this happens we can not run grouped reports because the control sheet scanned into the database without a special grouping code will not separate and group data.   4. Scan Sheets - Another error involves students filling in bubbles that are outside of the questions’ response range. For example, the question may ask students to respond using a scale of 1-5. In this case, they should fill in one of the first 5 bubbles. However, they sometimes bubble in the last 5 or the middle 5 on the ratings sheet. Forms that are completed in this manner should not be included in the packets as they will be included in the mean unless you specify an exclusion of those questions on the checklist.
  5. On-Line Surveys - Departments participating in the online surveys have not consistently given the checklist to Digital Campus and IRAP departments.
    If you are conducting your evaluations online, you must send a copy to Digital Campus MS# IT 121 Attn: Michelle Fisher and IRAP MS# TA 47.
