Checklist
All departments need to run the Student Ratings of Instruction Evaluation Checklist and department schedule number listing. The checklist schedule number listing and packet labels are crucial to IRAP when producing requested reports. Without them, we do not know which reports to run nor can we verify classes to be included. To find and run the SRI evaluation checklist in Peoplesoft, please follow the instruction here:
- Click on Reports Portfolio
- Student Administration
- Under student records
click on Faculty Evaluation Checklist, then click on the following:
- Search
- Report
- Enter the following information
- Academic Institution
- Term
- Academic group
- Academic organization
- Save
- Run
- Under select/check box
- "OK"
- Reports manager
- Keep refreshing until details appears
- Click on details and PDF file opens
- Checklist form (PDF)
- This checklist and schedule number listing must accompany the evaluation packets when delivered to IRAP.
- Please use the labels generated from Peoplesoft to place on each of your packets.

Control Sheet and Special Grouping(s)
To specify schedule numbers you want grouped to produce a separate mean – use column 6 of the Special Codes section when completing your scantron control sheet. The last digit at the end of the special code number tells the reporting software that this class is part of a special grouping. For example, if you want to group all schedule numbers of Plant 105W, bubble in 222223 on the control sheet for each section of that course. You can use up to seven special groupings. However, a single schedule number can only be assigned one special grouping. If you have more than one grouping then continue down the sequence of numbers ex. 222224, 222225 through 222229
The special codes that can be used are indicated below and shown on the checklist like so:
Special Grouping ___3 ___4 ___5 ___6 ___7 ___8 ___9
This information must be bubbled in on the Control Sheet accurately to compile your grouping. Please make sure the special code(s) is bubbled in correctly and corresponds to the grouping on the checklist, ex. if you check 4 on the checklist, then you must bubble in 222224 on the control sheet.
