  1. Updated list of  Tax Shelter Annuity (TSA) Companies
  2. CPIC Life/Medical Eye Services Claims
    • Vision Claim Form
    • Video Display Terminal (VDT) Claim Form
  3. Updated Unit 6 Trades Overview which includes the following Benefits update:
    • Effective July 1, 2004 , the CSU has agreed to provide long-term disability (LTD) insurance for eligible Unit 6 employees and pay the full cost of the premium to the LTD provider, The Standard Insurance Company. Certificates of coverage will be sent to all eligible Unit 6 (Trades) employees upon receipt of certificates in the Benefits Office from Standard Insurance Company.
    • Maternity/Paternity/Adoption Leave. Entitled to thirty (30) workdays of maternity/paternity/adoption leave with pay, which shall commence upon the birth or placement of a child. Once the leave beings, the days must be taken consecutively.
    • Rural Health Care Stipend – 2004-05. A one-time stipend of five hundred dollars ($500) will be paid to each eligible employee during fiscal year 2004/05 if criteria is met. Further information will be sent from the Benefits Office in separate cover along with Certificates of Coverage for Long Term Disability.
