Fee waiver application

Staff and faculty who plan to use their Fee Waiver benefits for the spring 2005 semester, must submit their fee waiver application to Employment and Benefit Services by Nov. 19, 2004.

Completed (and approved) fee waiver forms submitted to Employment and Benefit Services by the due date will be processed to adjust the student’s fees. Consequently, the student’s account will reflect the reduced fee to be paid by Dec. 10, 2004.

Late forms, improperly completed forms, or forms missing required signatures, will be delayed in processing. You may have to pay the full amount of fees upfront and request to be reimbursed for the difference or have the amount credited to your account. (You will need to contact the Accounting Office for reimbursements.)

All registration fees must be paid to the Cashier’s Office, Joyal Administration  Building, by Dec. 10, 2004. Payment not received by 5 p.m. will result in the cancellation of classes for the student by the Admission’s Office.
