Your Form W-2 for the 2011 tax-year will be mailed to the mailing address listed in the Personal Information Summary of Employee Self Service within MyFresnoState. Please review this information to ensure its accuracy. Any change of address must be entered into Personal Information by December 1, 2011 to ensure the change is made prior to the distribution of the 2011 Form W-2s. You may also submit an Employee Action Request to Payroll Services to update your address. We must receive this form by December 1, 2011 to reflect your address change in time.
