Grade Point Average Verification Form
If you are not currently admitted and enrolled at California State University, Fresno, the Scholarship Office must receive official verification of your cumulative grade point average from your last institution attended. Students need to submit the G.P.A. Verification Form no later than one week after the scholarship application has been submitted.
The Scholarship Selection Committees reserve the right not to consider any application if the G.P.A. can not be verified.
Click on the link below to print the form. After your authorized school official certifies your G.P.A., please mail (do not fax) the form to the Scholarship Office.
