Learning Plan

The Learning Plan is the official document that specifies the student’s responsibilities at their service-learning site and is required for all service-learning students.  The faculty member should make this form available to the student at the beginning of the semester. The faculty member will fill in the learning objectives on page 1 and the student will fill in the service objectives on page 1. The plan is signed by the student, faculty member and site supervisor, as indicated. The learning plan needs to be kept on file in the department for 1 year after the course ends.

Volunteer Application

Students who are serving at agencies that do not provide workers compensation insurance for volunteers have the option of filling out this application. Faculty should distribute this form at the beginning of the semester. These applications are signed by the student and turned in to the Civic Engagement and Service-Learning (CESL) office by the student.  It is not mandatory for students to fill out these forms, only if they want to be covered by the university’s workers compensation plan.  The faculty member does not need to collect or track this form in any way.  Applications are effective as soon as turned in to the Students for Community Service office and ideally should be submitted before the student starts their service hours.

Student Tracking Sheet

The student tracking sheet is an optional tool for students to use to help keep track of their individual service hours.  When students go to the community based organization site, they can fill out the log. This tracking sheet can be used by faculty to double check a students hours and by students to serve as proof of the hours they served at the community based organization.
